Children's Community Nursing

We provide nursing services for children and young people with acute and complex health nursing needs across Berkshire. This includes Specialist School Nursing. There are some differences in the services offered in the east and west of the county.

West: Wokingham, Reading and West Berkshire

Our Children’s Community Nursing West service covers Wokingham, Reading and West Berkshire.

We provide nursing services (treatment, care, support and advice) to children and young people in the community up to age 18 with complex, life-limiting and acute nursing needs. The nursing care can be provided within the child or young person's home, school, designated Local Authority short break settings or nursery.

We also offer specialist training in helping to care for your child's specific need for parents and carers within their own home, in special schools along side the Specialist School Nurse, and in designated local authority Respite Centres.

The team works very closely with the Specialist Schools Nursing team who provide health and nursing care to children attending these special schools:

  • The Avenue School, Reading
  • Addington School, Wokingham
  • Brookfields School, West Berkshire 
  • The Castle School, West Berkshire

East: Bracknell, Slough, Windsor, Ascot and Maidenhead

Our Children's Community Nursing East service covers Bracknell, Slough, Windsor, Ascot and Maidenhead.

We provide nursing services to children and young people up to age 18 with complex health needs and learning difficulties. The nursing care can be provided within the child or young person's home.

We also offer specialist training for parents/carers in helping to care for the child's specific need within their own home, in special schools, and in designated local authority short break settings.

We provide nursing care, advice and support to children and young people attending the following schools:

  • Arbour Vale School, Slough
  • Kennel Lane School, Bracknell
  • Manor Green School, Maidenhead
  • Priory School, Burnham, Slough

We are also able to provide support and training to school staff within these schools.

Specialist School Nursing service

The Specialist School Nursing service provides care and support to children and young people up to the age of 19 who have complex health needs and are enrolled in the following special schools:

  • Brookfields Special School in Tilehurst, Reading
  • Avenue Special School in Reading
  • Castle Special School in Newbury
  • Addington Special School in Wokingham

The Specialist School Nursing Team consists both of Specialist School Nurses and Specialist Nursery Nurses and is based within the four schools.

The team offers an open access service to all children, parents and carers and teaching staff to advise on health, development and medical conditions. They also work in collaboration with the school, the Children and Young People’s Integrated Therapy Team and other professionals to promote a holistic view of your child’s health and wellbeing. 

For children that need a specialised nursing healthcare plan, the Specialist School Nurse works with parents and carers to develop a plan covering your child's needs. When the care plan has been agreed with the parent or carer, the nurse will make sure that school staff are trained, so that your child can attend school safely.

The Specialist Nursery Nurses are an integral part of the team and provide individual care to children who meet the Children’s Continuing Care criteria, and need one to one support so that their complex health needs can be met safely during the school day.

Parent/carers

If you feel that your child needs a referral to our Children's Community Nursing team, please visit your GP. 

Professionals

We accept referrals from health service providers, education professionals and social care workers as long as the child or young person:

  • is under the care of a consultant and
  • has a nursing need and
  • is registered with a Berkshire GP.

If this criteria is met, please make a referral using the form below for the East or West.

Specialist School Nursing

Our service offers open access to all children, parents and carers and school staff enrolled or working in the four special schools.

Other professionals working with the children and parents and carers attending one of the special schools can refer to the Specialist School Nursing Team by completing an online referral form and we will contact you within seven working days.

Wokingham, Reading and West Berkshire
