Additional information documents

1. Please download the relevant additional information document:
   • Possible autism (from 2 years 6 months to 4 years 11 months at the time of referral)
   • Possible autism (from 5 years to 17 years 5 months at the time of referral) and possible ADHD (from 6 to 17 years at the time of referral)
   • Request to consider taking over ADHD medication (when child or young person has an ADHD diagnosis and is already taking ADHD medication)
   • Request to consider starting ADHD medication (when child or young person already has an ADHD diagnosis)
2. Please complete the form and save it.
3. Then proceed to the Berkshire Healthcare online referral form, complete it and upload this document when prompted.

Please note, we will not be able to proceed with the request for help unless you have uploaded this additional information.

When you are asked to upload the support plan, please upload either a copy of the support plan that is already in place, or a document summarising what support will be put in place.
